The children are our future. Yet a closer look at how we invest in that future needs further investigation and help. I have found that the mothers of the children who become community health workers are more willing to speak up about their children's lack of safe housing, need for supportive learning and after school activities. Finding their voices and a community from which to speak has encouraged many of them to think about getting their GED, going to a junior college or getting their citizenship.

Let us go through the journey in a small community anywhere in Texas, a very large state indeed, but with a lot of needs for families also. We see families that have arrived from anywhere in the world, not just from the Mexican Border, but what about the Canadian border, New York Harbor and the San Francisco harbor? But as Promotores de Salud, that is not our concern; out concern is to provide our communities with disease prevention education and health wellness and how to utilize the resources around them where ever they come from. How can Promotores de Salud relate to such issues? Maybe it's because they can relate to the issues since most of them live within the communities with the needs. What about the cultures, the languages in a country where all is diverse. As Promtorores de Salud we are just trying to make a small difference in the life of a community member that will contribute to our society in the future. Meeting community members halfway is better than letting them depend on the system all their lives. We have to realize that if an issues is in their community it is up to the community members to find the solutions to their issues utilizing all the resources possible. And the only form to provide the task, is to have Promotores de Salud in the front trenches, since we are trusted by th community, by going inside their homes.

Four students completed the first 80 hours of the CHW Instructor training offered via distance education. The students were located in various parts of the state. Each was in a different type of community. One student was located in the southeast part of the state. This is a very traditional area where families pride themselves on "being there" for several generations.

Another student was in a transitioning community in a larger city. The older residents had strong feelings against their neighbors who were "selling" out to the new kids, "that we do not even know."

A third student traveled from a large university site to an Hispanic community along the border with Mexico.

The fourth student was able to establish herself in an older community within a large metropalitan area that is also undergoing transition. The differences between the two transitioning communities is that in this community the older homes are not being torn down, they are instead becomes "expensive" rent property for people who can hardly afford it. Therefore the renters take little interest in the community because they do not plan on staying long. This is a temporary residences only. The long term home owners see their community losing property values because of the "new folk."

In each of these communities the CHW Instructors developed training sites so that members of the community could become certified community health workers and begin to undertake for themselves some of the problem solving that is unique to each site.

"I only had two people attend the training." How can I get more people from the community to attend?"

A question frequently heard from CHWs and CHW Instructors. A clue to answering the "attendance" question may be to identify how those present were ask to participate. Were they there because someone invited them, they had a interest, or they knew that their ideas were wanted, needed and would be included in the development process of a program, issue or opportunities to become a more active community participant and learner.

Participation means to include everyone within an open learning environment. Each participant is expected to do more than help with a community assessment. Their participation should be carried forth into the design, implementation and evaluation of the project or program.

An example of how partial participation operates can be fund in the use of focus groups. They are developed provides ideas for a project someone else will design, implement and evaluate, and then the group members disband.

An outcome for the participants should result in some identifable change for the individuals or community as well as keeping the participants in contact with each other.

A large hospital unexpectedly experienced a blood shortage for patients undergoing surgery for cancer. Then the hospital tried a novel idea, they asked the patients for help. By telling his story one patient was able to gather enough blood for all the surgeries cancelled at the same time as his.

Would this be a good example of community health workers at work?

The obstacles faced by health care providers and patients in rural areas are vastly different than those in urban areas. The National Rural Health Association has updated its “About Rural Health” fact sheet which contains an overview of data on rural and urban health disparities.
